Marching Band FAQ’s
Q. Where do I turn in forms and checks?
A. You can mail or drop your completed forms and check in the mail slot on my garage door at 1150 W. Knickerbocker Dr., Sunnyvale, CA 94087. Do NOT leave it on the porch. You can also come to the band room any time I’m there and leave it in my box, located in Mr. Burn’s office.
Q. The green uniform sheet says it costs $230 for Materials and Uniform fees. Do I pay it now or when we get fitted for a uniform?
A. The $230 annual fee is already included in the $640 Fall Season 2011 Cost amount (color guard, your fall season cost was adjusted to $410). As explained in the general information document, the $640 fall season cost amount consists of band camp, uniform, and competition costs.
Q. My doctor’s appointment is after the July 8th deadline for forms. Do I wait for the doctor’s appointment to turn in all my forms and payment?
A. Turn in all the forms you have with a note indicating when your doctor’s appointment is. We’ll expect the signed athletic activity form as soon as your appointment is done. If your doctor’s appointment is scheduled for AFTER band camp, we’ll need to complete additional forms to allow students to attend. Please contact Vicki Feltman at Vicki.feltman@gmail.com to discuss.
Q. I filled out the athletic activity participation form last year. Do I have to do it again this year?
A. This is a district form that is only valid for one school year. It needs to be completed every year. Check with your doctors - it may not be necessary to complete a physical exam before they agree to sign the form.
