Senior Awards Forms

Senior Awards Night is on Wednesday, May 29, 2013. It is an event showcasing your awards, honors, scholarships received from the colleges/universities you will be attending and community service rendered as a senior student. In this regard, make sure the following form is Completed and turned in to the College and Career Center no later than May 15, 2013.

Senior Awards Form

Board of Trustees Community Service Award  - 80 hours of community service from July 1, 2012 to May 1, 2013 with a non-profit organization(s); activities must be summarized on this form and the student must obtain a verification signature and contact number of supervising adult. For a list of accepted activities for students to earn the Board of Trustees Community Service Award Medal, CLICK HERE . Must be turned into the College and Career Center by May 1, 2013.
